Microtia is a congenital condition in which the outer ear is small, malformed, or missing. It can range from a slightly smaller ear to complete absence (anotia) and may be accompanied by hearing loss due to underdevelopment of the ear canal and middle ear structures.

Microtia surgery focuses on reconstructing the outer ear for a natural, symmetrical appearance and, when needed, coordinating hearing restoration in collaboration with ENT and audiology specialists.

Indications

Microtia surgery may be recommended when:

  • A child or adult has congenital microtia or anotia (complete absence of the outer ear)
  • There is associated hearing loss or middle ear abnormality
  • The appearance of the ear causes emotional or psychosocial challenges
  • A patient desires ear reconstruction for cosmetic or functional reasons
  • Previous ear surgery or trauma has led to loss of ear structure

Surgical planning typically begins in childhood (age 6–10), when the rib cartilage is sufficient for reconstruction and the ear has grown to near-adult size. Adult reconstruction is also possible and effective.

Description

Microtia surgery is a complex, multi-stage reconstructive procedure designed to create a realistic outer ear. Our approach is always customized to the patient’s needs, age, anatomy, and medical history. Surgery may involve one or more of the following:

• Creating an ear using autologous rib cartilage (your child’s own tissue)
• Using a medpor implant (biocompatible synthetic material) when appropriate

Shaping and positioning the ear with precision to match the opposite side
Improving symmetry and natural contours with soft tissue adjustments
Coordinating hearing evaluation and treatment with ENT and audiology teams

Techniques Used

1)  Rib Cartilage Reconstruction

• Performed typically between ages 6–10

• Uses the child’s own rib cartilage to carve and construct a natural-looking ear

• Multi-stage process over 2–3 surgeries to refine details and lift the ear from the scalp

2)  Medpor (Alloplastic) Reconstruction

• Uses a porous polyethylene implant shaped like an ear

• Covered with the patient’s tissue and skin grafts

• Typically completed in fewer stages and at a younger age (as early as 3 years)

Offers a viable alternative for families who prefer to avoid rib cartilage harvesting.

3)  Prosthetic Ear Options (Non-Surgical)

• For patients who are not candidates for surgery or prefer non-surgical correction

• Custom-fitted silicone prosthetic ears attached with adhesive or implants

At what age is microtia surgery usually performed?

For rib cartilage reconstruction, surgery typically begins around age 6–10 when the chest has developed enough cartilage. Medpor reconstruction may be possible as early as age 3. Adults can undergo microtia repair at any time.

How many surgeries are required?

Rib cartilage reconstruction often involves 2–3 stages. Medpor reconstruction may be completed in 1–2 stages, depending on the case.

Will the new ear look natural?

Yes. Our goal is to create an ear that is symmetrical, natural-looking, and in harmony with your child’s face. We use meticulous techniques to sculpt realistic details.

What are the risks?

Like any surgical procedure, microtia surgery carries risks, including infection, bleeding, scarring, poor wound healing, dissatisfaction with results, need for revision surgery, and implant issues. A comprehensive conversation with your surgeon should address these concerns.

Can hearing be restored?

While microtia affects the outer ear, hearing loss is typically due to issues in the ear canal or middle ear. We work closely with audiologists and other ENT specialists to explore hearing options or canaloplasty when appropriate.
